Targeted Case Management is provided by a licensed individual or agency that assists the person & support network to identify, develop, select, obtain, coordinate, utilize & monitor paid services & natural supports to enhance the person’s independence, integration, productivity & community inclusion consistent with the person’s needs & lifestyle preferences as outlined in the person-centered support plan.

Medicaid Title XIX:
Case Management Services, Inc. is a Targeted Case Management company affiliated with Johnson County CDDO and Cottonwood CDDO in Kansas. We serve individuals with intellectual and developmental disabilities who receive Title 19 Medicaid/KanCare. Medicaid/KanCare pays for our services for those who are eligible and choose Case Management Services, Inc. as their Targeted Case Management provider.
Case management services are comprehensive and must include the following (42 CFR 440.169(d)):
assessment of an eligible individual
development of a specific care plan
referral to services; and
monitoring activities
According to the Centers for Medicare and Medicaid Services (CMS), case management services must be “conflict-free” meaning there is a separation of duties from coercion.
